COMPERATIVE STUDY TO DETERMINE THE BEST DIANOSTIC IMAGE MODALITY IN PERIPHERAL ARTERIAL DISEASE (PAD) - turn up ExampleSimilar to pathology in the coronary arteries, the pathologic hallmark of the lesions involves plaques of atherosclerosis with calcium deposition, thinning of the media, patchy destruction of muscle and elastic fibers, fragmentation of the innate elastic lamina, and thrombi composed of platelets and fibrin (Duerschmied et al., 2006, 310-315). According to statistics delineated in different articles, the primary sites of involvement are the abdominal aorta and iliac arteries comprising 30% of symptomatic patients the femoral and popliteal arteries consisting of 80 to 90% of patients, and the more distal vessels, including the tibial and peroneal arteries making 40 to 50% of patients (McDermott, 2006, S2). Mechanically and topographically, atherosclerotic lesions occur preferentially at arterial branch points, which are sites of increased turbulence leading to altere d shear stress, and hence injury to the tunica intima. Epidemiologic and demographic data suggest that the distal vasculature is most commonly involved in elderly individuals, mostly in patients with diabetes mellitus (Marso and Hiatt, 2006, 921-929).The history and physical examination are usually sufficient to establish the diagnosing of peripheral arterial disease (PAD). An objective assessment of the severity of disease is obtained by noninvasive techniques. These include digital pulse volume recordings Doppler flow focal ratio waveform analysis duplex ultrasonography, which combines B-mode imaging and pulse-wave Doppler examination segmental pressure measurements transcutaneous oximetry stress testing, usually using a treadmill and tests of reactive hyperemia (Mohler, III, 2003, 2306-2314). Although the pace is fast, this could be because in the opening scene they got on with the play by using low camera angle shots. They choose to stick to the normal text draw for your s word heretofore though its a gun. In the first scene the mood was aggressive. However In the other scenes it was tenser and calm and more romantic due to its focuses on Romeo and Juliet. Also, on the opening scene the tone was harsh and deep to start with to develop the feud between the two sworn enemies.Overall the tone is soft and smashing to build more effect on their romantic love scenes. Their way of style was addressed in a 16th century way to follow the script of Shakespeare you could tell by the way they were dressing, who had high status and low status. Whereas the characters in Baz Lurhman wore modern clothes and more casual go to pieces to what people would wear now because the style and play was set in the 21st century. The first version of the Prologue was presented to us with a small old television with a blacked out background. The woman voice draws your attention in with its calm tone of voice, which tells us in a news form of way what the two sworn enemies are lik e and also a brief information about the two lovers Romeo and Juliet. The second version of the prologue starts with a chorus which describes the two sworn enemies in the city of Verona in an aggressive form of way. It flashes us clips about the city and the impressions the viewers are giving about Verona Beach is that the place is not a estimable place to be. During the flashing of clips u see a man been held down on a mattress by a police man.Every clips of it shows a gun. The city doesnt look like an Italian city it was made to look like New York. This prologue also lets us know that they catholic. The form of text they used was egg white text on black screen, using newspaper to show lines of the prologue. The sort of music played in this sequence was a dramatic music and orchestra music been played. Although in this prologue it was a male voice been used, the tone of voice was deep but told in bold written on a newspaper with characters pictures on it.The third version of th e prologue is presented very far and the sorts of expression shown on the face of the Capulet parents were sad and also were the Montague parents. The expression shown on Mercutio face was him feeling or put together to kill someone. In this sequences the two important characters were not shown this could be due to their innocents and both of them not seen each other as enemies. However Zefferelli version didnt expect a prologue the director just shows us a high angle get around of the Italian city and market, then shows the characters with a low angle short then you hear their voices.Images of violence and the characters are seen as gangsters. The presentation of the feud between the two families are you could see by the facial expressions that they dont like or agree with one another. Capulet ask for a fight then one of the Montague men was forced to draw for his sword then the fight gets more unspoilt and fights develops with pushing, shoving among peasants, then they ring th e sizeable bell to inform the others that theres a big fight then people get more involved. Sword fighting, rocks thrown and twisted above, vegetables thrown and wrestling to the floor.Tybalts character in the Zefferelli is betrayed as the leader of the gang. You see him without shot of his feet and moving up to his face with his gang behind him in a trigon formation which shows that is an important character. His also aggressive and hard hearted doesnt have any pity for no one apart from themselves. Hes also very mocking, likes making athletics and gesture of people. Whereas on the Baz Lurhman, his betrayed as a overbearing man and a low angle screen shot to show us his Cuban hillock shoes in a cowboy form of way but metal and big. Then the camera shows his clothes with a western music on the background which links to what his wearing. A leather jacket used to let us viewer know his religious because inside the jacket theirs a big image of Jesus on the inner clothe he was weari ng. In my point of view I think his vain and fashionable. His character is showing has confident, arrogant and sharp, you can tell by the way he holds his gun. He holds it in a calm and cool way, knows that his in control and a well know leader and a very good sharpshooter. Once is sword is drawn its something to be feared of.The way he also walks is cool calm and collected. The presentation of the two versions is both equally dramatic even though Zefferelli fight scene shows more chaos and bloodshed. But they both show the anger between both families clearly. The directors expectation is trying to capture young viewer especially student, trying to bring Shakespeare to life. In conclusion I would rather watch Baz Lurhman due to his brain of kids. He knew what kids of now days like so he made it spectacular, sexy and fashionable. Tim Adesunloro Mrs Hulbert.

Examine Conan Doyle Essay Example for Free

Examine Conan Doyle EssayExamine Conan Doyles The Speckled Band and Brian Moons Visitors. Discuss how far they are representatives of the emissary literary genre and how far they strike the attitudes to crime and punishment of the centuries in which they were written The Speckled Band and Visitors are two very different stories written in two different times. They were both(prenominal) written for different audiences, and thus the conventions of the genre have been interpreted in different ways over time. My essay will examine how far the stories conform to or diverge from the conventions and how far they relate to the crime situation of their time. The Speckled Band was written by the famous Sir Arthur Conan Doyle. It is a pre 20th century fabrication involving an unusual plot. A murder has taken place at a stately home and a young lady called Miss Stoner approaches Holmes for an answer. A strange plot twist makes Holmes think about the situation in a different light, but as normal the moorage is solved as soon as possible. Visitors is a short apologue written by Brian Moon. It is a very short bilgewater compa wild to The Speckled Band but in this time, a lot is achieved. The story is slightly back to front, with the robbers gentlemen, and the police louts.Burglary has been nationalised, and the Property Re-Allocation Technicians have visited the home of a young lady. There is no set story line, and the events occur at a rapid pace. The indication of a new genre of novels emerging was in 1842 when a short story, oddly called Who is the murderer? was released by Edgar Allen Poe. Poe highlighted and isolated the process in which the murder is unravelled the denouement. The conventions of the detective were first outlined in the book, and have been developed in further novels. The conventions of the novel are bid rules. They are what the reader expects to be in the book, and when exercised, make the book more enjoyable. If conventions are employe d, the book is easier to follow and understand, unlike Visitors. In Poes book the conventions of the detective genre are still sketchy, giving Poe a certain amount of Poetic License to mould them. A detective book should include the following. A crime, ordinarily a murder or kidnapping, drink downs the novel, normally to grip the readers interest in the story. A detective is then introduced, who is a normally a loner, with a comfortable background. He normally does his work because he feels that the police force is incompetent and do not do their job properly. A confidant, who is not as glistening as the detective is then introduced, he is more talented on the physical side of the investigation than the detective, often to the readers enjoyment. The investigation itself normally involves a red herring or plot twists to throw the detective off the track. Some of these are successful, and confuse the detective (and the reader) further. At the end of the story, the denouement is pro nounced, and the solution to the story is revealed. The books are written in such a way, as to involve the reader and to make him/her think try to discover the answer themselves. The conventions are illustrated in both stories, but are interpreted by the author in different ways, and with different meanings or consequences. The stories illustrate the changes in the justice system in the time between their writing. In the era of Conan Doyles story, the police force in the country had just been formed, and was still learning how to deal with problems. In comparison to this, in the age of Brian Moons story, the police force has become near unnecessary.Burglary has become legalised to combat unemployment, so the government has found that the police are in a nearwhat strange position. All they are required for is to take statements, and to verify a burglary has occurred. This means they can recruit people with no credentials. The Speckled Band has conformed to the conventions of the dete ctive story with very little divergence. I. e. the writer has looked at the rules of the genre and tried to make sure his story sticks to all of them. At first, The Speckled Band looks to be diverging from the rules, when the crime is not a murder, but a mystery. No weapon is found, and the cause of death is unclear. This is the first of the plot twists that we find. We meet the detective, Holmes, at the start of the story, and find that he is indeed, highly intelligent. This is illustrated by some of Watsons observations of his actions. following Holmes in his professional investigations and admiring the rapid deductions, as swift as intuition and yet invariably found on a logical basis are comments at the beginning of the story. This piece is written as if it were Watson telling the story of Holmes professional triumphs.Watson is manifestly in awe of Holmes and his brains, which leads on to the third convention, the detectives confidant. Watson is the assistant to Holmes, and i n compliance with the genre, he is clever, but not a genius. He is a practical man, and assists Holmes in the more physical side of detective work. Have your pistol ready in case we should need it Holmes comments. We (the reader) are constantly finding Holmes explaining things to Watson, and in some cases, Holmes becomes exasperated by Watsons lack of understanding. You see it Watson? But I saw nothing The police in Holmes view, will fail at everything. He is sozzled that his job is sometimes referred to as having connections with the police. Fancy him having the insolence to confound me with the official detective force After Roylotts departure from his office, Holmes illustrates his strength and annoyance at what he has done by bending the poker (previously bent by Roylott) back into its original shape. In other novels by Conan Doyle, Holmes is usually congratulated on solving the case before the police, by the police themselves Holmes sees the police in a very demeaning light, and assumes any case he chooses to investigate, the police have already failed.

White Noise by Don DeLillo Essay Example for Free

White Noise by Don DeLillo EssayWhite Noise is Don DeLillos break break fabrication that won the matter Book Award in 1985, catapulting the author into the elite circle of well-known postmodern writers. It deals with the main protagonists fear of death amidst the superficial consumerism that overwhelms America during the late twentieth century. The novel follows the life of Jack Gladney, a college professor in a school known as The-College-on-the Hill.He teaches Hitler stu eliminates, a discipline he invented himself, and lives jubilantly with his fifth wife, Babette, and his four children from different women. During this period, America is under outlet transformation wherein the nation develops strong attachment to materialistic values brought by the inevitable modernization. Jack and his wife argon caught in the web of industrialization that brings complications into their lives instead of comfort. The machines, televisions, supermarket and traffic noises creates an irritat ing sound that continuously hums in their head, compounded by the incessant whining of consumerism. The momentary luxury they subscribe from many various products is replaced by emptiness that lingers with them for a long time, and during this period, the protagonists develop a peculiar fear of anxious(p). In this early part of the book, Chapters 1-14, Jacks fear of death is gradually revealed it has been lurking in his mind as he observes that his life seems going too fast for him. Here, Jacks fear of dying is seen as not a because of the physical pain brought by death or the terror of being thrown into the eternal flame of hell rather, he is afraid to die because he washbasinnot leave everything behind. He earned his success. He worked hard to establish a prestigious status in his school, even going as far as doing silly things, like wearing elaborate robes in the school campus, just to solidify his credibility. He cannot allow a unpolluted dying to cut him from the glory he is enjoying on earth.Besides, he has a loving wife and healthy children that he is sincerely proud of. The first fourteen chapters do not show the entirety of the book, but they are enough to portray the patience of Jack as a father to his children and his dedication as Babettes husband, despite his previous four marriages to different women. All these cause him to dread death even more. This paranoia becomes a disturbing spectre that influences his judgments and rendering him uneconomical at times. He tries to find strength in Babette, who Jack describes as ample and reliable woman. In fact, Jack favors Babette over his previous wives because of her strong character. However, it turns out that Babette, too, is afraid of dying. Suddenly, their marriage is consumed by the spectre of death. They often talk about it over dinner and in almost every occasion. The question of who is going to die first is prevalent in most of their conversations, and soon enough, the topic becomes a part of their daily interaction. The issue has been looming in their thought all along, and when it is brought into open, things change among them and in their family. They become more preoccupied with the thought of death than with the welfare of their children. Thier disquiet over the issue dominates most of their waking hours that they tend to set everything aside. This novel is significant to the readers. It gives a moral lesson that death is a phenomenon that should not be feared about. It has been a part of human existence and must be accepted as such. One must not dwell too much on the thought of dying. It must be understood that it happens to everyone, regardless of race, age, sex or status. each human life ends in death and nobody can change it. Instead of focusing much on the thought of death, which gives nothing but worries and anxiety that can ruin happiness, the best thing to do is to enjoy life to the fullest. Another revelation of the story is that no material things in the world can mask the gloomy effect of death. Latest technology, impressive packaging of products and the flashiness of popular culture give momentary comfort, but they cannot conceal the terror of death. civilise Cited DeLillio, Don. Closure of a large amount of mental hospitals

Closure of a large numerate of affable infirmarysThere are numerous reasons as to wherefore much(prenominal) a large amount of rational infirmarys closed down, but to explore the reasons why they closed, we must look at the rise of the large institutions between the 19th century and the 1980s to hear its demise, why so m any an(prenominal) were built and why, for a small time period, they were successful. The intellectual hospital is defined as a hospital for the alimony and discourse of long-sufferings affected with acute or chronic psychological bronchiticness. Between the 19th century and the 1980s, the mental hospital has been defined as the criminal maintenancer for mental illness. But later on this g out of dateen age of care for mental illness, it has plow a victim of decareration ever since leading to a decrease in mental hospitals and its admissions, but why?The discharge of a patient was once known as a good thing as it symbolised the success of that hospita l through lesson interposition (later replaced by psychiatric negotiatement) but heretofore before this in that respect was a much harsher system with the rehearse of chains and straightjackets being the norm. As in the guinea pig of King George III who himself suffered from mental illness patients were non go outn as human beings, he was encaged, starved and beaten. The philosophy that therefore developed was the goal of a cure, to treat the patient in a therapeutic environment, To remove the patient from the midst of those circumstances under which insanity has been produced must be the number ace aim of interferenceAn entire change in the surroundings will some generation of itself lead to reco really (Bean and Mounser 1993, 4 quoting Busfield, 1986)The mental hospital really began in Victorian times hospitals were intentionally structured to be different from ordinary hospitals in terms of therapy, structure and location. (Rogers and Pilgrim 2010, 190). As where the general hospitals in the vicinity of a highly be area, a mental hospital would have been deliberately built in a place that is not so populated. The mental hospital (or sanctuary as it was once known) is seen as a conventional and humane way with dealing with the mentally ill one of the first legal acts to recognise this was the Lunacy fleck of 1845 which made local counties build asylums and gave the authority to detain lunatics, idiots and persons of unsound mind. (Rogers and Pilgrim 2010, 190). It was enforced and regulated by the Lunacy Commission. At the time the only place for the mentally ill to go to were workhouses and private madhouses. As mentioned before this moral treatment was in reaction of the harsh treatment normally given. One of the first institutions to use moral treatment was the York Retreat, The York Retreat was exposed in 1796 by the English Quaker community as a reaction against the harsh treatment utilise by other asylums. The belief at the time was th at the mad were wild beasts. The retreat was opened from The consciousness felt by a small throng of citizens of an overwhelming social evil in their midst (Rogers and Pilgrim 2010,198 quoting Jones 196040), although state-run asylums did not pick up this form of treatment for a while.Andrew Scull, an American sociologist argued that the mass amount of volume detained in asylums was a product of urbanization, industrialisation and master key forces during the first half of the 19th century and that detainment in asylums was a way to control social deviance.(Rogers and Pilgrim 2010,190) The ontogenesis in wage labour meant that services were not good enough to deal with this new form of social deviance meaning that the mentally ill could not be looked after by the family or local community, and around this time the stigma of how mental illness was a way out of humanity changed to the loss of self -control among the public. Meanwhile the French sociologist Michel Foucault sees th at this new undercoat market economy promotes rationality, surveillance and discipline(Rogers and Pilgrim 2010,190) But with this change of direction in the treatment of the mentally ill, many institutions began to fill up with patients rapidly overfilling local magistrates estimates, which lead to several extensions to a mental hospital. An example of this is the Conley Hatch mental hospital in London which opened in 1851 and shut down in 1993 and at its peak it held 3,500 patients. Many institutions promoted that they had the cure for mental illness, which led to a great surge in patients. In 1998, Gittens produced research into a mental hospital in Essex and followed the alive(p)s of staff and patients in the hospital and found many contradictions about mental hospitals. He found In relation to women patients it is clear for example that the hospital, based as it was on men-only and women-only wards constituted a women-only space and true asylum in a social condition in which th ere was little such space in external community life (Rogers and Pilgrim 2010, 191) He also found that there needs to be a restriction against out of doors forces such as social, economic and political conditions as they affect peoples abilities to deal with such material. As mentioned, its primary ideals were to treat patients with moral treatment yet this was forgotten about early on in its life, with the exaggerated numbers which local magistrates didnt expect and such serious illnesses, political bodies were press to keep costs down. Legal acts such as The Mental Treatment profess 1930 which allowed voluntary admissions to mental hospitals, it was also the act that got rid of the term asylum, and many mental hospitals found that it was not necessary to keep voluntary patients institutionalised. This led to a slight decrease in patients but it wasnt until the 1950s that its fall became apparent. Goffman (Rogers and Pilgrim 2010, 192) found that there were four types of institut ions because of thisNursing homes, where the incapable are cared forSanatoriums for those who have who have an unwanted threat to the community.Prisons, where the welfare of the inmate is not of paramount importanceMonasteries and convents for those who offer to be away from the worldIn 1948 the NHS was created, this led to all mental institutions being free for everyone, psychiatrists wanted this to broaden their field of study. The Mental Health Act of 1959 aimed to provide informal treatment for mentally ill patients with the possibility of detaining a patient against his/her willOne of the starting points of the crisis was in 1961, Enoch Powell, the minster of Health at the time, believed that mental hospitals were doomed institutions, so in 1961 he drafted The Hospital Plan where he planned to build thousands more hospitals and abolish certain mental institutes. The discharge of patients had become the policy of the demise of the asylum, subsequently the Community care blue bo ok was published which offered an alternative system to the mental hospital where mentally ill patients would see a specializer for an appointment just like a regular doctor, the patient could then carry on with their lives and live at home. The patient could live a fairly regular life and would stop the segregation from normal society it would entail the patient receiving treatment in a non-asylum setting such as in a district general hospital psychiatric unit. However it is argued that it is society that made the patient mentally ill in the first place, within the institutions there is a guaranteed market for experts services where specialists could hone their skills, yet the asylum grew out of a need to establish a social order. The asylum had many problems as Goffman points out, he believes in the mortification of self when being admitted to a mental hospital, with self being defined as being constructed by the pattern of social control which exists in an institution.(Rogers an d Pilgrim 2010,192) The persons past identity is completely stripped, their movement is restricted, hospital issue clothes are given and any personalized belongings are taken away, they are then obliged to disown their former selves through confessions with the staff, there is no such thing as covert and freedom of choice is extremely limited with all activities on a schedule.Wings (1962) research shows that feelings of withdrawal and apathy in patients was caused by their length of gentle in a ward and the lack of a stimulating environment, good medical leadership is not enough to reduce these feelings in patients, as he says it is unlikely that the functions of am energetic reformer can be built in to the social structure of an institution. As John Connolly points out once confined, the very confinement is admitted as the strongest of all proofs that man must be mad. by 1990 the average asylum held 961 compared to in 1930 which held 1221. Once the asylum had reached its peak si ze, it realised that the patient was losing their individuality through being guided by the institutions rules this resulted in a problematized re-entry into society. Originally the asylums were urged to be built by humanitarians, today the opposite is true, community care is now seen to be therapeutic and humanitarian, their return to the community legitimised community care as it deflected attention away from the demise of state responsibility for the seriously mentally ill and the current crisis of abandonment. (Bean and Mounser 1993, 8) During the time where many large institutions were on the brink of collapse Martin (1985) conducted research into what they were failing, he found that many of the failures were with the nursing staff whom participated in inhumane, brutal and threatening behaviour and committed mass negligence on their patients. He asked two questions. How do deft carers become to behave contrary to professional standards? And how have hospitals been arranged in such a way that abuse and neglect have not been prevented? To answer he found 6 points walloping institutions were situated outside mass populated areas meaning they were cut off from the communityWards were isolated from each other, and were almost their own little worldUn trained and un experienced staff were left to deal will large amounts of unruly patients,The worst wards were the ones rarely visited by a specialist, leaving the junior staff to do the work leading to a lack of leadershipThere was a lack of staff development through staff training courses etcThere was a huge lack of privacy for the patients(Rogers and Pilgrim 2010, 194) These factors led scholars to doubt that the large asylums could be put right and doubted that they could be reformed. They believe that there is a huge putridness of care.Ultimately every single patient will leave a mental institute whether they are ready to return into society or not and the goal is to cease contact with the hospital afterwar ds, scholars believe that the hospital, now in a community based system, has less of an importance it once had and is only one institution among others. The community based system is where the patient makes their first and last contact with treatment, they will enter the hospital for a short time and lodge their treatment in the community as their treatment does not require hospital admission. This process is known as deinstitutionalisation as patients will not be inured by hospital based treatment and instead will be by community based psychiatrists. However this is not new, in the old system the patient may have made his/her first contact with a G.P and then sent to a mental hospital for treatment, although this has not changed there are now community psychiatric teams to send the patient to the hospital and to treat them inside and outside the hospital, The old system had a sole digest on the hospital, today it no longer directs, controls nor dictates the pace of treatment or care (Rogers and Pilgrim 2010, 196)There is a whole array of reasons discussed as to the demise of mental hospitals, one reason is the pharmalogical revolution which suggests that the advances in medical treatments, such as tranquilizers let patients be discharged in mass numbers, the introduction of chlorpromazine in 1952 made it easier to manage disturbed behaviour, and therefore easier to open wards that had been locked, to engage patients in social activities, and to discharge some of them into the community Rogers and Pilgrim 2010,197 quoting Gelder et al 2001,769)Scull blames economic determinism and decareceation for this ample decline in the use of mental hospitals, he believes that it was state sponsored policy of closing down asylums, with the emergence of the welfare state, segrative control mechanisms became too expensive. Inflation contributed by unpaid patient labour and cost of employees, ex-patients also required care which had considerable costs, community care was the cheaper option. Another reason would be the shift of condense from chronic conditions to acute conditions such as depression and anxiety which has been described as the common cold for mental health specialists, these acute conditions were once treated by mental hospitals but it has shifted to the GP and is easily treated by them meaning that the majority of people with mental heath problems never seek specialist treatment. Additionally, as mentioned before community care played a significant role in the downfall of asylums, many specialists found that a change of shot was very beneficial to the patient, even patients with long-term chronic illnesses can return to society and live in community residences with good results making the mental hospital useful for a short period yet quickly and easily disposed of at point of departure no longer serving any purpose nor use to the ex-patient. However many studies have found that many psychiatric wards are non-therapeutic and that th ey share the same problems as they did in the Victorian era, principally because , according to Goffman, they act as a repair garage a problem is brought in, fixed and then sent out. Only medication is given, total care is not. A study conducted by Braginsky, Braginsky and Ring (1973) found that the minor patients all wanted to go home, while the more serious cases had no interest in leaving and instead maximised their comforts in the hospital as theyd rather stay in the hospital than become a victim of poverty outside. The focus has also shifted to the cause and solution of mental health, epidemiology (the study of patterns of health and illness in public health) and treatment were separated in the era of the mental hospital, today they are much closer, the hospital has disappeared leaving the attention to the inequalities in mental health prevention and positive mental healthTo conclude, the mental hospital has made leaps and bounds in the field of mental health with its aim to un derstand mental health scientifically and tried to cure mental illness, even though that was deemed impossible. Its failure was mainly due to costs and the shift of attention to other fields of treatment. Yet two types of institution has been created due to this, community mental health centres and district general hospital units both modern in their treatment. Although there are even so many similarities, high risk patients are still held against their will. Overall the care is much more professional, information is available over the internet, telephone or even the television, but more importantly there is a focus on the patients life and freedom, many may argue that the government still no longer offers a complete care system, but its a start.
